To make the process as smooth as possible, please ensure you have the following six required documents ready: 1. 📜 Official Birth Certificate (State-certified only; hospital certificates are not accepted). 2. 💉 Immunization Record (Florida 680 Form). 3. 🩺 Physical Exam (Must be dated after 8/11/2025). 4. 🏠 Proof of Residence (Signed lease or most recent mortgage statement). • Note: If you live with a homeowner/lessee, you will need a notarized "Homeowners Acknowledgement Form." 5. 💧 Utility Bill (Must match the address on your lease or mortgage). 6. 🆔 Government Issued ID.
