The Florida Department of Education is conducting a survey for parents of students who have an Individual Educational Plan (IEP), receive Exceptional Student Education (ESE) services, and are currently attending a Florida Public School. The survey is open through May 31, 2016. Please complete this survey at: http://www.esesurvey.com.
OPE Concept School registration is a lottery system. The registration takes place for one week at Orange Park Elementary and the lottery drawing takes place the following week at the Teacher Training Center, located on the campus of Fleming Island High School.
Orange Park Elementary will hold their open registration for the 2016-2017 school year March 28 - April 1. Registrations will be accepted from 8a-3p Monday through Friday, with additional hours, until 6:30 pm, on Thursday, March 31.
Please note, because OPE accepts students from all of Clay County, transportation is not provided. Also, all families must sign a commitment agreement. One of the OPE commitments is completing a minimum of 9 hours of volunteer work each year; volunteer hours may be completed by the parent or grandparent only.

Florida law requires all students have a Tdap (tetanus-diphtheria-pertussis) immunization before entering 7th grade. If you do not have a doctor or healthcare provider, the Florida Department of Health in Clay County offers FREE immunizations to students. For more information please call (904) 529-2800. Help your child have a smooth transition to 7th grade. Immunize now and return proof of Tdap shot (on the DH 680 form) to your child's elementary school prior to the end of school. Be Ready!
The 2015-2016 Title I Parent Survey is now open. The survey will be available online through May 31, 2016. This survey is open to all parent/guardians of children currently attending a SDCC Title I School.
